To enroll in Online Banking for the first time you need to sign an enrollment request. This can be done by stopping by any branch. You can also download the form below, fill it out, sign it, and fax it to Customer Service at 863-983-5043. Be sure to include an active e-mail address as the instructions to log in will be e-mailed to you. If you prefer you can also mail the form to P.O. Box 1237, Clewiston, FL 33440.
